In emergency situations, the clinician must judge the patient’s pain and psychological state to determine whether there is sufficient time to obtain interpretive services or begin to act on the patient’s behalf without informed consent. Particularly in the ER, interpreters are not always available due to the quick pace at which clinicians must operate. In general, hospitals tend to have a limited number of physicians and interpreters to assist. If clear communication is not established, appropriate care for the patient may not be provided. Clinicians work to use social cues, physical attributes, psychological state, and cultural values to help them determine the route of treatment to pursue. When a language barrier is present, the ability to utilize these cues is diminished. Although translating services are utilized in emergency rooms, the lack of extensive medical terminology knowledge by the translator can pose issues, especially in time-sensitive situations.
